Please Attach the Following:

IRS Tax Exempt Determination Letter
Operating Budget
Income & Expense Report
Balance Sheet Showing Assets and Liabilities
Form 990 Tax Return

Step Three: Evaluation and Site Visit

We will acknowledge receipt of your proposal. After reviewing your Request for Funding, we may request a site visit or conference call.

Step Four: Funding Decision by Eppes-Jefferson

Eppes-Jefferson utilizes a rolling application process. Requests for Funding are accepted and grant decisions are made throughout the year.
